The medical drama show ‘Transplant’ returns with a riveting season 4, plunging into the high-stakes halls of York Memorial Hospital. The protagonist, Bashir “Bash” Hamed, a doctor who immigrated to Toronto as a Syrian refugee, has recently found a new path for himself outside of becoming a regular surgeon. However, his newly unearthed mission to go after the ER fellowship position puts him in a competition of sorts with his ex, Mags. Meanwhile, Theo, whose professional decisions have recently led to the revocation of certain institutional privileges, deals with the aftermath of his new reality. Inversely, June enters an entirely new aspect of her career as she becomes an official doctor on the floor. Thus, as these individuals chart their respective challenges, lives are lost, but more importantly, saved. SPOILERS AHEAD!
Bash and Mags Work Side-by-Side to Save Two Lives
A level of awkwardness and tension persists between Bash and Mags, whose relationship recently came to an end. This charged dynamic between them is amplified as they’re both applying for a fellowship at York Memorial’s ER program. Nonetheless, both residents are on the case when the establishment receives two emergency patients, picked up from a hiking trail. One of the patients, John Doe, has one of his legs bitten off from the knee down, while the younger Jane Doe with him seems to be in shock of some kind. Initially, Mags comes to the wrong conclusion about the situation, believing the woman to be in danger. Meanwhile, Bash, who was momentarily seeing to other matters, tries to calm down Jane Doe, which leads to him getting stabbed. Even though it isn’t a significant wound, it remains a testament to the lack of synchronization in the emergency room.
As this chaos unfolds, another man, Simon, arrives at the hospital with a leg in a zip lock bag. Thus, the entire confusion around the situation is solved. The patients, Susan and John, and the new addition, Simon, are all biological siblings who went out into the mountains to bury the ashes of their shared father. The trek ended up resulting in a massive catastrophe when John’s leg got caught in a bear trap, pushing Susan to break it off with a stone to free her brother. As all of this happened, Simon fled to get help, leading to their eventual rescue. For the same reason, Susan had still been in a fight-or-flight mindset when she stabbed Bash shallowly. Ultimately, despite the confusion, the residents and their doctors, which now includes June, are able to rescue the pair.
Mags Gets Put on a Shortlist For a Heart Transplant
Mags’ persistent heart complication begins to negatively influence her life as she finds herself preparing for a life-changing surgery. Her heart arrhythmia condition had reached a point where outside intervention is required in order to contain the life-threatening risk it poses. Therefore, the resident is in need of a heart transplant in the future. Given the nature of the procedure, she can put down her name in a long list of other patients and wait for a possible donor to come around.
Furthermore, once Mags is informed of her turn, she won’t have a long time to consider her options and would simply have to submit to the treatment or risk missing out entirely. Naturally, this poses a daunting new reality for her, in which she has to prepare herself for a life-changing surgery while continuing to do her job at the hospital. For a while, she carries the weight of this looming change alone. However, at the end of her shift, after June shares her own unspoken problems about the added pressure of becoming an official surgeon, Mags opens up about her own condition. Even though June knows how to give the woman her space, she also asserts that as her roommate, she will be ready for any possibility that arrives in the future.
Bash Risks His Life to Save a Man From a Sinkhole
After Neeta reveals that York Memorial will only be accepting one residency at the ER due to a shift in the annual budget toward hiring more nurses, things become complicated for Bash. He’s eager to snag the spot for himself since the hospital holds so much emotional significance for him. Yet, with only one slot open and Mags as his direct competition, he knows he has a tough road ahead. Still, he tries to take his mind off the same during his time off from the hospital by going out grocery shopping for dinner-for-one. However, his attention diverts toward a crisis that seems to be unfolding in his neighborhood.
A giant sinkhole had opened up in the area, leading one man to get trapped underground. The firefighters are working toward clearing a safe passage for the survivor without sacrificing the city’s entire infrastructure. Yet, as evident through his communications via phone, the survivor, Kyle, has a collapsed lung, which may very well kill him before help can arrive. Consequently, once it becomes obvious that extracting him quickly won’t be possible, Bash resorts to desperate measures and sneaks into the sinkhole himself with a first aid pack in hand. As such, he finds himself treating Kyle by slicing an incision into his chest and draining out the liquid from his lungs, all in the darkness of the underground.
Bash’s reckless actions ultimately save Kyle’s life, as the two await evacuation while still stuck in the sinkhole. In the dire circumstances, the doctor reluctantly shares his story with his patient. He tells Kyle about how he and his younger sister, Amira, were able to flee from Syria with the help of a Canadian refugee-camp doctor, Elliot. The latter did everything for the duo, from filling out the right forms to raising money for their immigration and then allowing them a place in his home during their first few years in Canada. Yet, now six years down the line, he has faded away almost entirely from Bash’s life. Something about the unconventional rescue must remind the doctor of his past, as he seeks his old friend Elliot again once he and Kyle have been safely rescued from the sinkhole.
Theo Starts Seeing Clients From Home
Theo’s previous decision to report a patient’s sexual assault without her consent, and in fact in the face of her explicit demand to do otherwise, has had a significant fallout on his career. Although he can still see patients, he loses the privilege of working out of York Memorial, which forces him to hold virtual sessions from home. For the most part, this attracts patients looking to score easy prescriptions for some drugs. Initially, he attempts to fight for his job by trying to establish that he only reported the case because his patient was too scared to take the step herself. Nonetheless, after Bash helps him go through his own notes, he realizes the error of his ways and recognizes that he was deluding himself into a favorable reality.
Eventually, this helps Theo accept the consequences of his actions. Furthermore, he ends up getting a patient who actually truly needs his help on account of the neglectful treatment she has been receiving from the hospital, which is currently overflowing with an epidemic risk caused by a dentist’s convention. When it becomes obvious that the one-off paid session won’t be long enough to help her out, Theo ends up handing out his personal number to stay in contact with his patient. After a few preliminary tests, he directs her to York Memorial, where he tries to use his nonexistent influence to speed up her treatment. In time, her condition gets worse enough to require direct attention. Thus, he is able to quickly diagnose and direct the foundation of her treatment over the phone, ultimately saving her life. This experience helps the doctor realize he can still make a difference even at this dark time in his career.
